Elderly Couple Holding HandsThe Linn County Home Health program supports adults and families to function as independently as possible when health or social problems exist.

The goal of the program is to allow individuals to live where they choose, and if necessary, receive the appropriate support services and information required to maintain their quality of life - regardless of any disability or mobility problems they may have.

General Homemaker Program

The General Homemaker Program can help you when health problems limit your ability to do day-to-day activities. The program's direct care workers (DCWs) provide in-home, non-medical help to elderly and/or disabled people. Services are provided by professionally trained DCWs and supervised by either a nurse or a social worker in accordance with national standards and approved by the Iowa Department of Public Health.

General Direct Care Worker Services Include:

  • Grocery shopping and essential errands
  • Laundry
  • Life skills
  • Light housekeeping
  • Meal planning and preparation

Eligibility

If you live in Linn County and need help with at least two of the services Home Health provides, you are eligible for the program. 

Delivery & Fees

Referrals to the Linn County Home Health Program are accepted in person or by telephone, mail, fax or email. Service delivery is prioritized according to need. Fee for service is assessed on a sliding scale according to household income and resources.
