[ARCHIVED] Together! Healthy Linn Seeks Community Input for Community Health Assessment

Every 3-5 years, the Together! Healthy Linn collaborative led by Linn County Public Health assesses the health needs and assets of our community through a Community Health Assessment (CHA). A community survey is one tool that is used as part of the assessment process. Everyone who lives in, works in, goes to school in, or visits Linn County is invited to complete the survey, which is available here: Community Health Survey (arcgis.com). Completion of the survey will help gain insight on issues impacting the Linn County community’s health and wellbeing. 

Many organizations are involved in Together! Healthy Linn, ranging from hospitals and clinics, non-profits, and government agencies. These organizations are working together to complete the assessment. After completing the assessment, the group will create a Community Health Improvement Plan, which identifies action steps to address the priority issues from the assessment. Once the assessment and plan are complete, the organizations intend to continue the partnership to implement the plan and track progress.
